Brendon R. Connolly, M.D.

Brendon R. Connolly, M.D., FAAOS
Dr. Connolly specializes in Sports Medicine, Shoulder Surgery and Arthroscopy. He received his bachelor's degree in biology from the University of Miami. He graduated with honors and cum laud and is a member of Phi Beta Kappa. In 1997 he graduated from the University Of Miami School Of Medicine at the top of his class and is a member of Alpha Omega Alpha. Entering the army after medical school, he completed his internship in general surgery at Madigan Army Medical Center in Tacoma, Washington in 1998.
Upon graduation from the United States Army School of Aviation Medicine, Dr. Connolly was stationed in Fort Greeley, Alaska for a year as a Flight Surgeon and Family Practitioner in support of the 360th aviation squadron. He returned to the Pacific Northwest and completed his orthopedic surgery residency at Madigan Army Medical Center in 2003. He then spent four years as Chief of Orthopedics and Physical Therapy in Fort Sill, Oklahoma. It was here that he cultivated his interest in sports medicine, Arthroscopy and shoulder surgery. After 10 years of active-duty service, he left the army in 2007 and joined Sarasota Orthopedic Associates.
He received his board certification in Orthopedic Surgery in 2005. Dr Connolly was the first Orthopedic Surgeon in Sarasota to earn the Certificate of Additional Qualification (CAQ) in Sports Medicine, a subspecialist board certification, in 2009.
Dr. Connolly has authored many articles published in peer-reviewed journals such as the Journal of Trauma; Spine; The Journal of Hand Surgery, and The American Journal of Orthopedics. He is Double Board Certified in Sports Medicine and General Orthopedic Surgery and is a Fellow of the American Academy of Orthopedic Surgeons.

Dr. Cottom received his bachelor's degree with honors from Michigan State University. In 2002 he graduated Magna Cum Laude from Scholl College/ Chicago Medical School, in Chicago, Illinois. Subsequently, he completed a three-year surgical residency in reconstructive and trauma surgery of the foot and ankle at the Detroit Medical Center/Wayne State University, in Detroit, Michigan focusing in all areas of Foot, Ankle, and Leg Surgery. Dr. Cottom is also extensively trained in the conservative and surgical treatment of diabetic conditions affecting the lower extremity including limb salvage.
Dr. Cottom is honored to be one of only a few podiatric surgeons in the United States and the only in the state of Florida that has ever completed an accredited American Orthopedic Foot & Ankle Society 12 month surgical Fellowship. This advanced surgical training focused on foot and ankle trauma, reconstruction as well as total ankle replacement. This was completed in Columbus, Ohio under the direction of Gregory Berlet MD, Thomas Lee MD and Terrence Philbin DO and was affiliated with The Ohio State University. This is the same type of fellowship that MD/DO orthopedic surgeons complete if they wish to sub-specialize in the foot and ankle.
In 2005 he completed an AO International Trauma and Reconstructive Foot and Leg Fellowship under the direction of Dr. Sigvard Hansen MD in Seattle, Washington. Dr. Cottom also completed a Fellowship in Kurgan, Russia focusing on the Ilizarov technique for correction of acute trauma and complex deformities of the lower extremity in both adult and pediatric patients.
Dr. Cottom is currently the only Orthopedic Fellowship Trained Foot and Ankle Surgeon in the region who is trained on 4 FDA approved Total Ankle Replacements. Not every patient is a candidate for this type of treatment. During a short visit in the office all options can be explored.
Dr. Cottom has authored numerous publications in the area of Foot, Ankle, and Leg surgery and has won research awards both at the state and national levels for his work. He is very active in lecturing as well as in teaching the latest surgical procedures to other surgeons. He is a devoted patient advocate who strongly believes that the quality of education and training make a difference in treatment outcomes. Dr. Cottom is a member of the Education and Scientific Affairs Committee for the ACFAS which helps deliver superior continuing medical education to foot and ankle surgeons in the United States. He also serves as the Fellowship Director of the Sarasota Orthopedic Associates 12 month Foot and Ankle Fellowship. A native of Michigan, Cottom now lives in Siesta Key, Florida with his wife Devin.

Dr. Diaz received his Bachelors Degree Cum Laude from the University of Puerto Rico and his Medical Degree, also Cum Laude, from the University Central Del Carribe School of Medicine in Puerto Rico. He completed residency training in Physical Medicine & Rehabilitation at the University of Texas Health Science Center, at San Antonio with special training in EMG & nerve conduction studies and pain management and sports medicine. He joined Sarasota Orthopedic Associates in 2001.
Dr. Diaz is a board certified member of the American Academy of Physical Medicine and Rehabilitation, the Association of Academic Physiatrists and the Physiatric Association of Spine, Sports, and Occupational Rehabilitation.
W. Kim Furman, M.D.

Dr. Furman specializes in Arthritis and Joint Replacement Surgery. Dr. Furman joined the practice in 1987. He has served as Chief of Orthopedics at Sarasota Memorial Hospital, Doctors Hospital and Health South Rehabilitation Hospital of Sarasota. He engages in Orthopedic Research as well as publishing articles in medical journals. He has traveled to centers in Asia lecturing and instructing on total joint replacements. He received his undergraduate degree at Northeastern University in Boston, his medical degree from Mt. Sinai School of Medicine in New York, his surgical internship at Mt. Sinai, and his fellowship in Total Joint Replacements/Arthroscopic Surgery at the renowned Anderson Orthopedic Research Institute in Virginia.

Dr. Michael J. Gordon graduated with a Bachelor of Arts degree in Economics from Yale University in 1992 and obtained his medical degree from Cornell University Medical College in 1998. He completed his residency training at Baylor College of Medicine and subsequently completed a 12 month Orthopedic Hand Surgery Fellowship at the Hospital for Joint Diseases in the New York University Medical Center.
He specializes in treatment of the upper extremity. From the fingertip to the shoulder, he is well versed in the management of arthritis, trauma and other problems of the upper limb. He treats compression neuropathies such as cubital and carpal tunnel syndrome. He also performs arthroscopy of the joints of the upper extremity and is trained in the latest techniques for treating fractures of the shoulder, arm, forearm, wrist and hand.
A large proportion of Dr. Gordon's patients are children and Dr. Gordon and his staff enjoy caring for Sarasota's pediatric population. Dr. Gordon treats both upper extremity and general pediatric orthopedic conditions.
Dr. Gordon also has a background in medical research. He has spoken at national orthopedic surgery meetings and has been published in several peer-reviewed journals.
He is Board Certified by the American Board of Orthopedic Surgeons and by the American Society for Surgery of the Hand. He has also received the Certificate of Added Qualification in Hand Surgery.
Dr. Gordon is a member of the American Academy of Orthopedic Surgeons, the American Society for Surgery of the Hand, and the Sarasota County Medical Society.

Dr. Howard is board certified and specializes in Lower Extremity Orthopedic Trauma and Reconstructive Surgery. He has completed an advanced Fellowship training program in Complex Foot and Leg surgery through Sarasota Orthopedic Associates and Dr. Harold W. Vogler in 2003. He received his medical degree from Barry University, with his undergraduate B.S. Degree and Masters Degrees and study in electrical engineering and advanced mathematics from The University of West Florida. Dr. Howard also served as an aerospace engineer prior to medical school from his training at Purdue University. Currently he is involved with the Fellowship program in Complex Foot and Ankle Surgery in conjunction with Dr.Vogler & Sarasota Orthopedic Associates. Dr. Howard is published as a contributor in a foot & ankle surgical textbook.

Dr. Morgan received a B/A degree in chemistry at Grinnell College in Grinnell, IA. He then received an M.D. from Howard University College of Medicine in Washington, D.C. He served an internship at Passavant Hospital in Chicago and an orthopedic residency at Northwestern University in Chicago. Dr. Morgan completed a fellowship in pediatric orthopedics at Children's Hospital in Cincinnati, OH. He also received an MBA degree at the University of South Florida, Tampa, FL.
Dr. Morgan has practiced orthopedic surgery in Evanston, IL and in Gary and Merrillville, Indiana for over 30 years. He founded an orthopedic group and holds faculty appointments at Northwestern University. He has also practiced in Sarasota since 2005. Dr. Morgan is past-President of the National Medical Association and the Indiana Orthopedic Society. He is the author of numerous publications and has made over 200 scientific presentations at local, state and national courses and conferences.
Dr. Morgan has been recognized for his contributions to medicine and society by several institutions. Among his many accolades are the following. He was named "Orthopedic Resident of the Year" in 1972 by Northwestern University and "Physician of the Year" by the National Medical Association, Northwest Indiana Chapter in 1986. In 2005, Dr. Morgan received the Martin Luther King Jr. Legacy Award for National Service and the Joseph Pitts Award for Community Service from the Gary Branch of the NAACP. He has also received the Distinguished Alumni Award from Howard University College of Medicine in 1999.
He is pleased to have joined Sarasota Orthopedic Associates in May of 2007, where he is practicing general orthopedics with a special interest in pediatrics, joint replacement and treatment of sports injuries.
Donald J. Slevin, M.D.

Dr. Slevin specializes in Sports Medicineand disorders of the knee and shoulder. He joined this practice in 1982. In 2007, Dr. Slevin discontinued his surgical practice and began concentrating on non-surgical conservative treatments of orthopedic disorders. He graduated Cum Laude from Tufts University with a Bachelor of Science degree and continued on to earn his Medical Degree from the Chicago Medical School. He did his surgical internship at the Maricopa Medical Center in Phoenix and his orthopedic residency at the Phoenix Integrated Orthopedics Program. He completed his Fellowship in Arthroscopic Surgery at St. Luke's Hospital in Phoenix.
He has traveled to various centers in the U.S. and Canada and worked with international experts in their fields, thereby advancing his expertise in the field of knee and shoulder disorders and arthroscopic techniques.
He qualified for the Olympic Trial in 1972 in Track and Field. Dr. Slevin has won gold and silver medals as part of the U.S. Macabiah Team in Israel in 1973 and was appointed to the All-American Collegiate Track and Field team as well. In 2002, Dr. Slevin qualified to participate in the Senior State Track Championships where he placed first in his District and eighth for the State of Florida in the 100 yard dash.

Dr. Vogler received his bachelor's degree from Henderson State University in Arkansas. In 1972, he received his Doctorate in Podiatric Medicine from the Illinois College of Podiatric Medicine, in Chicago. His first residency in Reconstructive Surgery of the Foot & Ankle was completed at Harrison Hospital in Mt. Clemens, Michigan. In 1981 he completed an A-0 & Traumatology Fellowship at the University of Freiburg, in the Federal Republic of West Germany and Switzerland.
In 1983 Dr. Vogler was Professor of Surgery and Chairman, Dept. of Surgery at Pennsylvania College of Podiatric Medicine and also Chairman. of the Dept. of Foot & Ankle Surgery & Traumatology at St. Joseph's Hospital in Philadelphia.
Dr. Vogler was visiting Professor of Orthopedic Surgery and Advanced Research Fellow in Reconstructive Surgery & Traumatology of the Foot & Leg at Malmo General Hospital-Lund University in 1990-1991. He was also appointed as Research Fellow in Biomechanics & Functional Anatomy at the Panum Institute of the University of Copenhagen during this same time frame.
Dr. Vogler has authorized 71 publications and textbook contributions in all aspects of Reconstructive & Trauma Surgery of the Foot, Ankle and Lower Leg. He has directed two Advanced Residencies in the same specialty area and has been active in the educational sphere since 1974. He has held academic rank in Sweden, Denmark & the USA. In 1992, Dr. Vogler relocated to Tampa, FL where he worked at The Foot & Leg Medical Center as a consultant surgeon. He joined Sarasota Orthopedic Associates in 1999 after a long academic career and has 38 years of experience in Orthopedics, Paralysis, Fracture & Reconstructive Surgery of the Foot & Leg. He was Chairman of the Section of Foot & Ankle surgery at Sarasota Memorial Hospital from 2008 until Fall 2011.
